The original formatting of the references was very poor and inconsistent from chapter to chapter. Little effort was made to correct these problems, but the information is generally sufficient for the diligent reader to find the original sources. A number of editorials or anonymous articles in magazines were cited. These will be listed below under “editor” or the magazine title.
